Policy
We have the following policies in place:
We cannot provide refills of medications provided by other providers or other medical conditions. We are not required to prescribe any medications on your first visit.
Controlled Substances: We very rarely prescribe other controlled substances for specific such as phobia, panic attacks, or short-term sleep aids. We do not prescribe benzodiazepines for frequent or long-term use, and we do not prescribe opioids.
FMLA, disability, emotional support, and other paperwork request: Sometimes, patients encounter circumstances that impair their ability to function adequately to meet their job requirements. At times, it may be appropriate to use FMLA or disability to provide support until the function has returned. We do not complete this paperwork for new patients whom I do not have an established therapeutic relationship with. For individuals requesting this type of assistance, I require that we meet to discuss the specific functional impairments. Please allow up to 5 business days for completion of this.
Cancellation Policy: Each appointment is reserved for you. If you need to cancel or reschedule, we request at least 24hr notice from your scheduled session time. This allows the opportunity for someone else who needs to benefit from the availability. If you fail to give 24-hour notice or a No-show, you are responsible for the full cash rate of your appointment or a minimum of $100. If you are more than 15 minutes late for an appointment, you will be considered a No-show and charged accordingly. For example, an appointment on Monday at 2 p.m. needs to be canceled by 2 p.m. on the Friday before. Please note that insurance companies will not reimburse for missed or canceled appointments.
Refill Policy: Medication prescriptions should be requested during our sessions together. This allows us to discuss how the medications work and how long you should take them. Occasionally, you may need refills between sessions. Please allow up to 5 business days for medications requested outside our appointments. I can not authorize refills if you have no future appointment since I am legally required to ensure that you are in active treatment if I am prescribing medication.
Confidentiality: Information discussed during the course of treatment is confidential. By law, information concerning your treatment may be released with your consent (or that of your guardian if applicable). In the event where there is suspected child abuse, neglect, exploitation, or an imminent danger to harm one’s self or others, the law requires the release of confidential information. In these instances, we are required to make a report to the appropriate authorities.
